Overview

The Resource Coordinator is responsible for the assessment, planning, and coordination of care through provision of education and support to patients and their families by connecting to community resources to ensure quality patient outcomes and appropriate utilization of healthcare services.

Responsibilities

  • Assesses patient needs, coordinates services and referrals 
  • Provides education to patients of all ages based on knowledge of growth/development across lifespan
  • Create/update personalized care plans for patients
  • Makes referrals as necessary to resolve emotional, social, or financial problems impacting health care status
  • Utilizes tools and documents that support a guided care process, collaborate with patient/family toward an effective plan of care
  • Promote healthy behaviors in all populations and ensure navigation assistance with community resources
  • Attend assigned trainings and meetings 
  • Collaborate with patients’ families, friends, and social supports in developing care plans
  • Linking patients to social services programs and entitlements such as transportation assistance and interpretation services
  • Conducting regular follow-ups with patients to evaluate progress, promote continuity of care and ensure improved health outcomes
  • Evaluates effectiveness of interventions continuously and modifies plan as needs develop
  • Maintain records of care coordination/case management activities in EMR as appropriate

Qualifications

  • Work requires a basic knowledge of social and health related resources in the community. 
  • Must have knowledge of the problem solving process and of medical and community settings.  This level of education is normally acquired through completion of the social work, psychology or related field at the bachelor’s level. LBSW preferred
  • 2 years of case management, transition planning, or other relevant experience preferred
