Overview

The RN Care Coordinator utilizes professional nursing skills to assess patient/family scenarios for clinical appropriateness for the service setting. The key goal of the RN Care Coordinator is to work collaboratively and proactively with patient/family, physician and the healthcare team to implement an, individualized plan of care to meet the needs of the patient/family’s across the continuum that optimizes quality of care, utilization management and patient satisfaction. 

Responsibilities

  • Assesses, re-assesses and analyzes the medical record according to specific criteria using pre- established screens for utilization review, quality assurance, discharge planning, infection control, and risk management from admission through transition.

  • Recognizes appropriate criteria for medical necessity and level of service and provides information to the physician advisor when criteria is not met. Follows appropriate protocol for timely notification of patient and/or family to seek appropriate level of care.

  • Uses various computer programs to input data to be analyzed and collated for administration and medical staff reports.

  • Assesses all new admissions noting high risk issues when applicable. (New diagnosis, chronic disease (s), Domestic Violence, Adult/Child Abuse, Readmission).

  • Understands/Identifies Standards for Case Management

  • Understands/Identifies standards/procedures to prioritize needs/objectives to design concise Case Management care plans.

  • Understands and is able to coordinate implementation of services to meet needs, evaluate and modify plan as necessary and develop appropriate follow up.

  • Obtains and evaluates relevant information (medical, psychosocial, functional, vocational and financial) through interviews with all parties.

  • Advocates on the patient/family’s behalf for the services necessary to meet the established plan/goals. Education and support is provided to encourage patient empowerment and self-reliance.

Qualifications

  • Follows established hospital policies and procedures. 
  • Current lndiana RN licensure required.
  • Complete knowledge of the clinical setting, the skills involved in nursing interventions and the nursing process. This is normally acquired after three years of clincial experience at RN level. 
  • Bachelor's degree is preferred. 
  • 3-5 years of clinical leadership experience preferred. 
  • Certification in Case Management (CCM or ACMA) preferred. May take up to 5 years to complete after completing exam pre-requisites. 
  • Previous Utilization Review or Case Management experience preferred. 
  • Computer proficiencies to enter and retrieve data, to assemble and organize data, to use, analyze and integrate date from diverse sources.
