• The MSW Care Coordinator provides support to patients and families in dealing with psychosocial issues related to illness or injuries including terminal conditions.  
• The MSW Social Worker collaborates closely with the medical and nursing staff, interpreting psychosocial data and coordinating the transition planning and health care continuum of services among inpatient units, ambulatory settings, and providing assessment/crisis intervention in Emergency Department to facilitate appropriate disposition.

• Requires knowledge of social, behavioral and health-care related resources in the community.
• Requires knowledge of the problem-solving process and of medical and community settings.
• Requires Masters of Social Work with current Indiana licensure.
• Prefer one year experience in health care or behavioral health care setting.
