• Assesses the physical therapy needs of patients and families in the home environment.
• Develops plan of care for the patient in conjunction with the physician and other health care providers.
• Administers prescribed care according to physician orders.
• Educates patient family/caregivers on plan of care and provides support to the patient and family/caregivers.
• Works with support services in obtaining needed supplies, equipment and services for the patient.

• Requires specialized knowledge as to administration of treatments and maintenance of equipment.
• Must exercise judgment in administering and analyzing treatments.
• Education requirements are a Bachelor of Science degree and a diploma from AMA approved and accredited school of physical therapy.
• Successful completion of state board examination required.
